The One-Hour Phone Consultation with Dr. Lopez includes The Health Assessment Profile (HAP), which is an in-depth 4-page health questionnaire that evaluates each persons own history, symptoms, diet, stresses and exercise routine. From this we can better understand where the problems are coming from.
The HAP will help us uncover what is at the root of your problem and get you on the right track.
